WASHINGTON, D.C. – Senator Jay Rockefeller, Chairman of the Finance Subcommittee on Health Care, today announced $935,000 for 17 community health centers in West Virginia to provide more coordinated care, modeled after patient-center medical homes. The community health centers will each receive $55,000.
"Providing coordinated care in a medical home setting benefits both the patient and the physician," said Rockefeller. "This funding will support community health centers across the state to improve care and services. This is another tangible benefit for West Virginians from the health reform law."

The funding for these grants comes from a provision in the Affordable Care Act and is administered by the U.S. Department of Health and Human Services.
